Do you know a Veteran who...

  • Lacks a fixed, regular and adequate night-time residence?
  • Has had to leave their old residence for reasons beyond their control?
  • Is staying in a shelter, hotel, park, abandoned building?
  • Is sleeping in their car?
  • Is facing eviction?


  • A member of a Veteran family:
  • A Veteran
  • A member of a family in which the head of household or the spouse of the head of household, is a Veteran
  • Low-income: House-hold income does not exceed 50% of area median income as established by HUD (Housing and Urban Development Government Agency).
  • Homeless or at imminent risk of becoming homeless.
  • Discharge status on DD214 must be above dishonorable

The Intake Appointment is essential to determining your eligibility in the program

  1. State Photo ID Card for Veteran and Spouse
  2. DD214
  3. VA medical ID card (if you have one)
  4. Birth Certificate (for all minor children in household)
  5. Social Security Card (for everyone in household)
  6. Marriage Certificate
  7. Lease Agreement and/or Eviction notice
  8. All Income Statements (SSI, SSDI, VA)
  9. Most current Checking and/or Saving Account statements
  10. MLGW bill or account number (if you have ever had account)
  11. SNAP and/or TANF Award letter
  12. Receipt and/or letter from where you have been staying (Shelter, Hotel, etc...) with:
  • Place’s Name
  • Veterans’ Name
  • Length of time at place/program
